That doesn’t require a mod, though they could also have been using one coincidentally.
Ive never seen anyone achieve those numbers legitimately through normal vanilla play, every screenshot ive seen with E tier numbers involve a mod that calculates all possible moves picking the mathematically best one probably also cherry picking a favorable seed.
Is it possible for a super card-counter analytical person to play balatro perfectly or lock down a seeded run to get to those numbers? Sure. Is it likely the average gamer™ is going to without the help of a mod to brute force calculations? Mmmm…
I’ve gotten e through normal play. It isn’t hard with the right combination of jokers. You’re making it seem harder than it is
