Hmmm. Patient is a vampire. Likes his blood sweet. Think that is the whole joke, but not sure.

The doctor ask their patient about their blood type (A, B, O, or AB), but the patient responds unusually by saying they like their blood sweet, thinking about their “type” as in preference (For example, “what type of girl do you like?”).
